Salary: £35,992
- £37,536 per annum pro rata + £7,000 MRS per annum pro rata
Hours: 37 Hours per week (7 day Service) and Part time available
Base location: Working from home and any CBC offices, including Priory House, Shefford and various Hospitals across Central Beds. You will be expected to attend a CBC office location as often as needed to carry out your role, which will be no less than once per month.

More about the role:

We are looking for an experienced Registered Social Worker, Nurse or Occupational Therapist to join our professional and dedicated Hospital Discharge team. We provide a 7-day service, operating in a hybrid working pattern with rest days in the week, and are open to providing a job-share system for employees who just want to work weekends. Our goal is to create a strong network of supportive individuals who thrive in a fast-paced environment. This role will suit someone who is eager to get things done and make a real difference in their community.

 

The role will involve going into hospitals, resident’s homes, nursing homes and residential care homes to maximise people’s independence, with a strong focus on improving their futures. We need people to be flexible, have experience of working in these environments, and approach every day with a ‘can do’ attitude.

 

Your main responsibility will be to focus on the people you care for and what is right for them. You’ll cover the whole of Central Bedfordshire and out-of-area hospitals, and manage your own caseload, which will include supporting a set number of people through discharge pathways (from hospital and discharge to assess). You will also work closely with the reablement team, who are the first port of call out of hospital.

 

Our aim is to support people at the most vulnerable point in their lives to get home and recover safely, consequently becoming as independent as possible.

 

We are looking for the following qualities in the successful candidate:

  • Motivated and committed to the role.
  • Team player who is willing to help out their colleagues when necessary.
  • Strong communication skills – you’ll need to speak to people at the most vulnerable time of their lives.
  • The ability to work under pressure.
  • Be knowledgeable around paying for care.
  • A recognised social work, nursing or Occupational Therapy qualification and be currently registered with Social Work England, Nursing & Midwifery Council or HCPC .
  • Access to a vehicle and hold a full, valid, driving licence.

 

This is a very diverse role where every day is different and presents a new challenge. Some of the areas of work you can expect to be involved in are:

 

  • Duty function: discharge to assess (going out into the community – visiting residents in their homes, care homes, hospital, etc.).
  • Home recovery service.
  • Care act assessments – working within legislation and latest hospital discharge guidance.
  • Mental capacity act assessments.
  • Discharge planning.
