Community Assessment Officer

Team:  COA - Hospital Discharge Service

Family: Providing - CBG7
Salary: £28,598 - £31,022 per annum pro rata
Hours: 37 Hours per week

Location: You will be expected to attend a CBC office location as often as needed to carry out your role, which will be no less than once per month.

Contract Type: Hybrid - Permanent

About the role:

As a Hospital Community assessment officer, you’ll be at the heart of our Better Care Fund developments whilst supporting our residents to leave hospital safely. Join us and find your greatness in supporting vulnerable people to be safe, well and as independent as possible.

 

Our Hospital Discharge Service provides Assessment and Case Management across all client groups. We are committed in supporting people to leave both acute and non-acute hospital settings with timely and safe integrated discharge planning.  Central Bedfordshire has a number of positions to support people across multiple hospital services that serve our Council area. The service works in close partnership with other health and care systems, we work alongside our Community Health Service provider to deliver a multi-disciplinary approach to Integrated Health & Social Care. The working week of 37 hours is spread across 7 days on a rotation base.

 

We are currently looking to recruit an community Assessment Officer to join our busy and multi-skilled Adult Hospital Discharge Team.  The Team is committed to supporting people to be as independent as possible with a strong focus on improving individual outcomes in a strength based, person centred way, the use of Integrated TEC and creative solutions. You will be expected to complete holistic assessments as required by the Care Act 2014.

Adult Services is fully committed to working collaboratively with our health/partnership organisation colleagues to provide a seamless multi-agency response in supporting residents with their health and social care needs. Our collective practice has a strong focus on early prevention and optimising levels of independence. The role is currently combined with working within one of the area satellite offices and some collaborative time with your team in the office and working from home. You will be required to complete face to face appointments with persons receiving the services and carers and where appropriate through visual virtual technology.
