Homecare Brokerage Co-ordinator

Team: Brokerage, Contracts and Compliance Team

Job Family: Providing - CBG7

Salary:  £28,598 - £31,022 per annum pro rata

Hours: 22 hours per week (Mondays, Tuesdays and Fridays)

Location: You will be expected to attend a CBC office location as often as needed to carry out your role, which will be no less than once per month.

Contract Type: Hybrid - Permanent

About the role:

The role is based within the Brokerage Team in the Adult Social Care & Housing Directorate. 

The role of Brokerage Coordinator is a key conduit enabling people to be supported in their homes with an appropriate care package in place – in a timely way. The postholder will be working with the Council's external care providers, social worker and hospital teams to allocate care through the agreed protocols. 

Purpose of the role

To co-ordinate and prioritise incoming requests for the placement of homecare packages for all CBC customers. Working with the team to ensure the service is delivered efficiently and in line with current guidelines, to be the first point of contact for support in relation to the Brokerage functions. 

Main duties: 

  • Book care packages for homecare with care providers in accordance with the funding agreement and the Homecare Framework.
  • Co-ordinate and prioritise incoming requests for care packages
  • Input, maintain and reconcile records relating to all adult service care packages on the Council’s Adult Social Care system CareDirector.
  • Ensure providers are paid accurately and promptly
  • Manage the statistical information daily basis on the activity of Brokerage
